Our IDD Home Care Program
Our IDD Home Care Program is like no other in New York! Our leadership has over 30 years of experience with agencies like OPWDD, OMH, and the DOE. A Person/Family-centered approach assesses whether the child or adult is getting all the services they may need. We collaborate with the CCO-Care Managers, OPWDD providers, or the other agencies involved with the individual’s care to maximize all available support. Specializing in both pediatrics and geriatrics, Extended is there to help patients of any age get the in-home services they need. We accept most insurances!
