  • Job Description: Extended Home Care needs an AP Specialist.  The Accounts Payable Specialist will perform all accounts payable functions including vendor files, invoices, check preparation and data entries.  Additional responsibilities include: maintain vendor's database; reconciliation of invoices and purchase orders; data entry of invoices into accounting system on daily basis; prepare weekly accounts payable aged invoice reports and weekly payment schedules for approval; prepare and print checks for payment; code accounts payable items appropriately to ensure proper entry into the financial system and handle all vendor correspondence via phone or email.

  • Job Description: Provides skilled nursing on assigned patients according to medical orders, policies and procedures.  Provides assessment of newly referred patients, assesses health needs and eligibility for home care services.  Provides patient and family teaching prior to discharge and on an on-going basis.  Develops and implements treatment plans in conjunction with the home care coordinator, physician, and patient / caregiver.  Supervises home health aide according to the agency policy.  Reassesses patient's needs for service on a continual basis and communicates any changes in patient's status with physician and home care coordinator.  Conferences with home care coordinator to assure continuity of care.  Documents accurately and completely in patient's clinical record.  Submits written documentation on timely basis in accordance with agency policy.  Makes referral for other services as needed.  Plans, facilitates, and documents patient's discharge from the agency.  Maintains updated professional knowledge and participates in patient education programs to ensure optimum quality of patient care.  Performs other nursing activities as directed.

Nurse Coordinator                                                    Open

  • Job Description: Plans, implements, manages and evaluates the provision of both professional and ancillary home health services to ensure that all patient's needs are met and quality care is provided in accordance with Federal, State and Agency guidelines.  Establishes and updates the care plan with written input from the patient/caregiver, physician and other appropriate multidisciplinary health team members.  Completes all required documentation in a timely manner including interdisciplinary referral form, 485, OASIS, interim physician orders and coordination notes according to Agency Policy.  Manages the activity of the multi-disciplinary team providing care to patients.  Ensures that all visits scheduled and provided are authorized and covered by physician order.  Reviews reports from all involved disciplines, evaluates ongoing patient care needs and communicates those needs to the physician.  Maintains proficiency in clinical and administrative skills.  Demonstrates sound judgment and independent problem solving skills in order to initiate appropriate intervention with regard to patient's psychosocial and / or physical impairment.  Facilitates the care of the patient in the home settings by utilizing appropriate community resources, counseling and teaching patient / family and advocating on behalf of the patient.  Communicates case load and patient care issues to the Nursing Manager.  Updates the Home Health Aide plan of care, communicates with Home Health Aide Coordinator.  Develops, implements and carries out a discharge plan in conjunction with the Primary field, patient / caregiver and members of the health care team.  Interprets agency policy to patients/families.  Ensures that all visits made have prior authorization if required by patient's the managed care insurance company.  Work collaboratively with managed care insurance company.  Monitors the quality of therapeutic services through written and verbal communications with all disciplines.  Participates in performance improvement activities, team meetings and orientation as requested.
